Insurance Matters

We participate with the following major insurance providers:

• United Healthcare insurance
• Mutual of omaha PPO
• Coventry PPO
• Humana
• Tricare

We participate with additional unlisted insurance carriers.

The bookkeeping and insurance departments request the patient bring their current insurance card each time they come to the office so that each person's insurance information is complete and correct. Doing this will greatly help in getting insurance paperwork done in an efficient and timely fashion.

Many insurance policies require patients to make a co-payment at the time of each visit. These co-payments must be paid at the time of each appointment.
